ADULT RELEASE OF LIABILITY AND INDEMNIFICATION AGREEMENT
PLEASE READ CAREFULLY ***THIS AGREEMENT LIMITS OUR LIABILITY
In consideration of being allowed to participate in any way in ice hockey or on or off ice activity at the Kevin Bell Arena or with Homer Hockey Association, its programs, and its related events and activities, I, the undersigned, acknowledge, accept, and agree that:
The risk of injury from the activities involved in this ice hockey and on or off ice activity or event is significant, including the potential for permanent paralysis and death, and while particular skills, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist.
I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of Homer Hockey Association, its officers, directors, employees, volunteers, or others, and I assume full responsibility for my participation.
I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the immediate attention of Homer Hockey Association.
RELEASE OF LIABILITY: I affirm I am physically fit to participate. I, for myself and on behalf of my heirs, assigns, devisees, personal representatives and next of kin, release and discharge Homer Hockey Association, its owners, parent, affiliated and subsidiary organizations, together with all of their respective shareholders, directors, officers, members, managers, landlords, lessors, employees, volunteers and agents (the released parties), from any and all claims, liability, demands, actions and causes of action, whether for property damage, personal injury, disability, or death, arising from my participation in any aspect of ice hockey and other on ice or off ice activities or use of facilities owned or leased by Homer Hockey Association, whether based on negligence of the released parties or otherwise, to the fullest extent permitted by law.
INDEMNIFICATION: I hereby agree to indemnify, defend and hold harmless Homer Hockey Association, its owners, parent, affiliated and subsidiary organizations, together with all of their respective shareholders, directors, officers, members, managers, landlords, lessors, employees, volunteers and agents (the released parties), against any claim, liability or cause of action of any kind, whether based on negligence otherwise, for property damage, personal injury, disability, or death, made against the indemnified parties and arising from or related to the participant’s participation in any aspect of ice hockey or other on or off ice activity with Homer Hockey Association.
COVENANT NOT TO SUE: I hereby covenant and agree I will never bring, authorize to be brought, or otherwise participate in any claim, suit, or cause of action of any kind, whether based on negligence or otherwise, for property damage, personal injury, disability, or death, against Homer Hockey Association, its owners, parent, affiliated and subsidiary organizations, together with all of their respective shareholders, directors, officers, members, managers, landlords, lessors, employees, volunteers and agents (the covenantees), arising from or related to the participant’s participation in any aspect of ice hockey or on or off ice activity with Homer Hockey Association.

CODE OF CONDUCT
Sportsmanship
Fair play will be emphasized and considered essential for all HHA players, coaches, parents and spectators. Spectators are not to interfere with any officials. Referees carry the authority to stop play to inform coaches of unacceptable spectator behavior. Coaches will then have the responsibility to correct the situation. It is the referee’s decision to suspend play.
Language and Behavior
Profanity, abusive language, and obscene behavior are not allowed during HHA on or off-ice functions, and will be strictly enforced through USA Hockey Rules.
Facility Upkeep
HHA facilities have been built and are maintained by volunteers with donated time, effort and funds. It is the responsibility of each player, parents, and spectators to respect the privilege of using the rink. If something needs attention, please report to the rink staff.
Zero Tolerance
In an effort to make HHA activities a more rewarding and desirable experience for all participants, HHA prohibits all types of physical abuse, sexual abuse, emotional abuse, bullying, threats, harassment and hazing, all as described in the USA Hockey SafeSport Handbook. Participants, employees or volunteers in HHA may be subject to disciplinary action for violation of HHA policies or for engaging in any misconduct or abuse that violates the USA Hockey SafeSport Policies
